The chapter chair (or other chapter organizer) should communicate with the student(s) in the chapter to determine who will be available to perform at the Honors Recitals in June, as only these students are to be reported on this form.
You will need the following for each Honors Recitalist:
- the email address and phone number of the parent/guardian
- the name and composer of the piece of music
- all teachers’ name and email addresses
- if required, the collaborative artists’ names and email addresses
